EMDR notes fail in a specific way when a general tool writes them: the ratings vanish and the procedure gets invented. Cogent names the phase by number, keeps the cognitions and the ratings verbatim, and leaves blank whatever you did not record.

01What the draft keeps

The elements a EMDR note has to carry.

Pick EMDR at the start of a note and the draft comes back with this structure, using eye movement desensitisation and reprocessing terminology precisely and leaving blank anything you did not record.

  • The phase named by number as well as name, for example Phase 4 (Desensitisation), which matters for accreditation and supervision.
  • Target memory, image, and the negative and positive cognition as stated.
  • SUDS and VOC at the start and at the end, kept as the numbers you recorded.
  • Bilateral stimulation type, whether eye movements, taps or tones.
  • Channels of association, blocking, looping, abreaction, dissociation and any interweave used, when you recorded them.
  • Closure status, including whether the target was left incomplete.
02Why a general tool gets it wrong

A note that splits the difference fits no one.

The failure mode here is confabulation. A general model knows what an EMDR session usually looks like, so it writes the assumed close, the check-ins and the self-regulation that a protocol implies, whether or not any of it happened. In medico-legal work that is a real exposure. Cogent was corrected on exactly this during beta and now preserves what you documented and marks what you did not.

Does Cogent record SUDS and VOC ratings?

Yes, at the start and at the end, kept as the numbers you recorded rather than summarised into words. An EMDR note without the ratings is not much use in supervision or in accreditation logs, and losing them was one of the first things beta testers flagged.

Will it name the EMDR phase?

Yes, by number and name together, for example Phase 4 (Desensitisation), rather than the vaguer reprocessing phase. That was a specific request from an EMDR supervisor in the beta programme because accreditation records are read phase by phase.
