Notes in your approach · Assessment

AI notes for psychological assessment sessions, written the way Assessment writes.

A first session is not a therapy session written shorter. Cogent drafts assessment notes with mental-state observations, history at the right depth, and an initial formulation framed tentatively.

01What the draft keeps

The elements a Assessment note has to carry.

Pick Assessment at the start of a note and the draft comes back with this structure, using psychological assessment terminology precisely and leaving blank anything you did not record.

  • Mental-state observations recorded as observations rather than inferences.
  • History calibrated to what matters rather than exhaustively transcribed.
  • An initial formulation framed tentatively, opening the work rather than closing it.
  • Risk documented explicitly, including when there is no active risk.
  • What is still unknown, marked rather than smoothed over.
02Why a general tool gets it wrong

A note that splits the difference fits no one.

Generic tools write a confident formulation from a first meeting, which is the wrong clinical posture and reads badly if the note is ever disclosed. Assessment notes need to hold uncertainty visibly, and to say plainly where information is still missing.

03Common questions

The questions psychologists ask before they try it.

Does Cogent write diagnoses?

No. Cogent does not diagnose, interpret assessment data, predict risk or recommend treatment, and that boundary is part of its registered intended purpose as a Class I medical device. It documents what you observed and recorded, and you remain the author of record.

What happens where information is missing?

The draft marks the gap rather than filling it. Missing values are left blank with a marker naming what is absent, so an incomplete note reads as incomplete instead of quietly reading as finished.
