Measures your client completes by link, scored and filed for you.
Send a link, the client fills in the questionnaire on any device, and it comes back scored and filed in their folder, ready for the report. Or enter the totals yourself for the measures you score on paper. Either way, Cogent does the arithmetic and leaves the interpretation to you.
Send the questionnaire, and it returns scored.
Generate a secure link for a measure, send it to your client however you already reach them, and they complete it on any device with no login and nothing to install. The moment they submit, Cogent scores it and files the result in the client's folder, and you get a notification that it is ready to review. The link expires after twenty-one days and works once.

Every form carries crisis signposting, and tells the client plainly that it is a questionnaire rather than a monitored service. The client link is on every plan.
For the measures you score on paper, enter the totals and Cogent tabulates them.
The measures used across neurodevelopmental and general work are built in. Enter the subscale totals from the scored protocol and Cogent computes the headline score, the bands where a published band exists, and the subscale breakdown, filed against the folder. The raw items stay on the paper protocol where the copyright keeps them.

On the Professional plan you can upload a photo or PDF of a completed, scored questionnaire, and Cogent reads the numbers into the entry form for you. You confirm every value before it is saved, so a misread never slips through.
However a score gets in, it is typed once and carried into the report.
A filed score sits in the client's folder and flows into the findings of an assessment report when you draft one, so the same row of numbers is never entered twice. The scoring is deterministic arithmetic; the meaning is yours to write.
The client answers, and Cogent holds only the numbers, encrypted.
The client is not asked their name on the form and sees nothing identifying about themselves or about you. What Cogent stores is the set of numeric answers, encrypted in transit and at rest on UK infrastructure and linked to the folder rather than to a name. You read the total and decide what it means; the client is never shown a score or a result.
