02Why a general tool gets it wrong

A note that splits the difference fits no one.

A general-purpose tool writes a competent summary of a conversation, which is not a CBT note. It loses the agenda structure, blurs a Socratic dialogue into narrative, and reports that homework was discussed without recording what was set. Reviewers and supervisors read for exactly those elements.

Can AI write CBT session notes?

It can draft them for you to check and sign off, which is a different thing from writing them. Cogent takes your shorthand or the session transcript and returns a structured CBT note with the agenda, the cognitions elicited, the techniques used and the homework set. You remain the author of record and nothing is filed until you accept it.

Will it invent content that was not in the session?

No. The drafting is constrained to what you actually recorded, and where something a CBT note usually carries is missing, the draft marks the gap rather than filling it with plausible text. That behaviour came directly out of beta feedback from clinicians who found other tools confabulating standard procedure.
