Cogent writes a finished note into the client's record in Cliniko or Halaxy, authored by you. You choose which client, and nothing is sent until you press the button on that document.
Three steps, and the middle one is yours to decide.
Connecting is a one-off. Linking a folder to a client is a decision you make per client. Sending is a decision you make per document.

Connect once.
In Settings, paste the credentials you generate from your own login in Cliniko or Halaxy. Cogent checks them with your provider straight away and confirms it is ready. For Halaxy you also pick yourself from the practitioner list, so every note it files carries your name.

Point a folder at the client.
Open the client folder, choose Edit, and search your client list in Cliniko or Halaxy. You pick the record. Nothing is matched for you, because a folder label is not enough to be certain which client is the right one.

Send the finished note.
A finished draft in that folder gets a Send button beside Copy and Export. It writes the note into that client's record, authored by you, as a finalised note with your headings kept.
Setting it up takes about ten minutes, and I will do it with you.
Generating a key is a few clicks in your own practice software, though the wording is unfamiliar if you have not met it before. There is a step-by-step walkthrough for Cliniko, Halaxy and the extension, and for now I am setting this up individually with anyone who would rather not do it alone.
The note is authored by you, because it is written with your own credentials.
Both systems take credentials you generate from your own login, and both record a practitioner as the author of a clinical note. So what lands in the record carries your name, and the note reads as yours because it is: you reviewed it, so you wrote it.
Your records stay where they already are.
Your client records already live in your own practice software, under the agreement you already have with that provider, and that is exactly where they stay. A note from Cogent is your own clinical work going back into your own system, filed against a client you chose, when you ask for it.
Cliniko and Halaxy are live. The next one depends on what you use.
