Notes in your approach · ACT

AI session notes for ACT, written the way ACT writes.

ACT notes turn on process rather than content, so a note that records what was talked about has missed the session. Cogent records which processes were in focus and what moved.

01What the draft keeps

The elements a ACT note has to carry.

Pick ACT at the start of a note and the draft comes back with this structure, using acceptance and commitment therapy terminology precisely and leaving blank anything you did not record.

  • Hexaflex processes in focus, named as processes rather than as topics.
  • Defusion techniques used and how the client responded to them.
  • Values clarification work, kept in the client's own words.
  • Committed action agreed, including the size of the step.
  • Workability framing, which is the frame the whole session is judged against.
02Why a general tool gets it wrong

A note that splits the difference fits no one.

A general tool reports content, and ACT is a process model. Defusion gets written up as challenging the thought, which is the wrong model entirely and reads to any ACT supervisor as though the session did something it did not do.

03Common questions

The questions psychologists ask before they try it.

Does Cogent know the difference between defusion and cognitive restructuring?

Yes, and it is one of the clearest examples of why an approach-specific template matters. Defusion changes the relationship to the thought and restructuring evaluates its content, and a note that conflates them describes an intervention that did not happen. The ACT template holds the distinction.

Can it help with a Choice Point formulation?

Choice Point is on the roadmap for the formulation diagrams, added after ACT-practising beta testers asked for a client-facing alternative to the hexaflex. Session notes in ACT conventions are available now.
