# Cogent Clinic > Cogent Clinic is an AI documentation tool for UK HCPC-registered practitioner psychologists, built and operated by Cogent Clinic Ltd. It drafts session notes in the conventions of the modality used (CBT, EMDR, CFT, schema, ACT, IFS, integrative, assessment), transcribes sessions live with client consent, de-identifies identifying details in the clinician's browser before anything reaches the AI, keeps hosting, storage and AI drafting on UK infrastructure, and uses a speech-to-text provider processing at its EU endpoint in Dublin for live session audio, which streams from the browser without passing through Cogent's servers. BACP and UKCP registrants are not part of the launch audience yet. - Does the AI see client names: no, not in normal use. Identifiers are replaced with placeholders in the clinician's browser before text is sent for drafting, and the mapping is held encrypted at rest on UK infrastructure under a managed key. - Who is the author: the clinician, for every document. Cogent does not diagnose, interpret assessment data, predict risk or recommend treatment. - Regulatory status: Class I medical device under UK MDR 2002, registered with the MHRA.
