Cogent Clinic is a product of Illuminated Thinking Ltd, a private clinical practice registered in Scotland. The core team is one clinician and the engineering partners needed to build the thing responsibly.
The practical problem was ordinary. Session notes take too long. A good note is a specialist piece of writing — it's not a brief, it's not minutes, it's a clinical record. The AI tools on the market looked like they could help, but every one of them surfaced the same set of unanswered questions: where does the data go, who trains on it, what jurisdictions does it cross, what happens when something goes wrong. For a clinician with registration to protect and a regulator to answer to, those questions are load-bearing. The sales pages rarely address them.
So we built a product that does. It keeps patient-identifying content out of the cloud entirely. It runs inside the UK. It has a hash-chained audit log you can verify. It tells you honestly what it doesn't do — diagnose, triage, make clinical decisions. The clinician is the author of record for every document produced, and that sentence is in the system prompt as well as the terms.
We are in Glasgow. We are small. We would rather ship something respectful of the profession than something that “scales” at the cost of it.
